Texas Heart Institute:THIでORに出入りするようになって、2週間が経ちました。
来週あたりからOn-Callも始まりそうです、、、。
THIでは、前日に翌日の手術予定症例が各Resident/Fellowに割り当てられます。
10部屋のCardiac ORと、2部屋のHybdrid ORで、毎日10−20例の心臓血管手術が行われています。
今週から、幸運にも開心術の第1助手も当ててもらえるようになりました！
（患者の右側は、まだまだ"Far side"ですが、、、）
THIでの一週間は、
Mon：late start dayで8:00に患者が入室
（ということで、6:00〜Fredaの早朝英会話レッスン）
Tue：通常通り7:00入室
Wed：6:00〜Cardiac Surgery Educational Lecture
（text bookは"Cardiac Surgery" by Kirklin 4th edition）
Thu：7:00〜M&M (morbidity and mortality) conference
Fri：通常通り7:00入室
Sat/Sun：On-call以外はお休み！
術後管理は麻酔科医と循環器内科医がメインで行います。
基本的には手術室でのHands-on trainingに集中できる、という外科医にとっては幸せな環境です。
On-callはResidentとFellowが組んで、4−5日に一回の割合で回ってきます。
（当然、当直手当は付きませんし、外勤もありません、、、）
とにかく、手術中のSurgeonとのCommunicationも含めて、まだまだ不安がいっぱい、、、。
が、泣き言言っていても誰も助けてくれないので、一生懸命やるしかない！！
I've been at THI for 2 weeks right now. Elective cases are distributed a night before surgery to residents and fellows. Fortunately, a few open heart surgical cases have been distributed to me this week. Unfortunately, I will be involved in an on-call schedule starting next week. On-call is likely to come every 4-5 days.
In ICU, Anesthesiologists and Cardiologists are taking care of postoperative patient mainly. So basically, residents and fellows can concentrate on surgical hand-on training in OR.
Anyway, I still am anxious about my English communication, especially with surgeons during operations. Although nobody can help me in OR, I'll do my best, and try to have fun!!
