Welcome back to Houston!

やっと妻と子供達が、日本からHoustonに帰ってきました！

この1ヶ月、友達も居ないHoustonで、一人で寂しかったよお！！
親友である息子のSanくんと、その友達で娘のSaiちゃん、そして愛する妻が帰ってきてくれて、これで何とかバカ暑いHoustonでも頑張れそうです！
（どういうわけか、娘にとってはパパは、まだ刺身の付け合わせ程度の存在のようです、、、）

Baltimoreに居たときよりもちょっと忙しくて、相変わらずちょっと貧乏ですが、Houston Summerを家族でEnjoyするぞ！

ということで、いつまで経っても、一人を楽しめないMasaでした。

3年ぶりの日本を満喫して帰ってきました！
日本を離れたときは3歳だった2人が、今は6歳。
何か感慨深いですなあ、、、。

My wife and children finally came back from Japan to Houston! 
I'm a little busier in Houston than in Baltimore, and we are still as poor as in Baltimore, but we can enjoy Houston summer!!

はじめてのOn-Call

今週、はじめてのOn-Call (当直) をしました。
まだ病院の電子カルテにも不慣れなのに、大丈夫なの？と不安でしたが、、、。

基本的には病院に泊まって、何かあるとPager (ポケベル) が鳴ります。
電話でのやり取りだけで済ますには、USの臨床現場での、知識も経験も英語力も足らず、結局3回に1回は、ベットから起きて足を運ぶ羽目に、、、。
ともあれ、急患や急変も無く、何とか無事に、初On-Callを乗りきる事が出来ました。

次の朝、ORのResident/Fellow Assignment Sheetを確認すると、Masaの名前がありません、、、。
そうなんです！Post-Call (当直の次の日) はお休みなのです！！
ホントに帰っていいの？？？

ということで、日頃忙しくて見られない、Dr. Coselliの大血管手術や、面白そうな手術症例をのんびり見学。
でもって、昼過ぎにお家に帰って、韓国系食料品マーケットへお米と味噌を買いに。
夕方は、Running & Gym Workout。

俺、長生きしちゃうよ！？

まったく、よく鳴るPagerだ！

This week, I was an "on-call" for the first time in the US. During the night, I was woken many times by my pager , and a third of these messages I had to personally attend to at ICU and other wards because of my lack of clinical experiences, knowledges and English communication skills. As for clinical English conversation, it is still tough for me. But I was
able to complete my " first on-call assignment" without any problems. Whew,,,.
Next morning, I could not find my name on residents/fellows assignment sheet. Yes! I'm a day off as "post-call"! So I enjoyed watching interesting operations as observer. I also enjoyed shopping at Korean market and gym workout! 

Mmmm, it's going to be awesome!


 

Lab Home Party

Texas Children Hospital Cardiac AnesthesiologyのDr. BradyのLab Partyに招待してもらいました。

Dr. Bradyは、Johns HopkinsでのBossであったDr. Hogueと共に、今のResearchのFoundationを築いた人で、3年前にJohns Hopkins HospitalからTexas Children Hospitalに移り、現在はClinical & Animal Lab Researchを精力的に行っています。

今回はResearch Stuffとその家族が30人程集まり、素晴らしいPool付きの豪邸でBBQ Partyを楽しみました。

Medical StudentやDoctor、そしてその家族と色々な話ができて、とても楽しい時間でした。
そのなかに、以前Johns Hopkins Cardiac SurgeryのLabで会ったことがあるMedical Studentがいました。

”久しぶりMasa！何してるの？”
"7月からTHIでCardiovascular Surgical Fellowしてるんだ"
"そうなんだ。僕のおじいちゃんが、昔THIでSurgeonだったんだ”
”、、、。君のLast Nameなんだっけ？”

聞いてビックリ！
彼のおじいちゃんとは、THI:Texas Heart Instituteの創設者であり、その名を知らない心臓外科医はいない、Dr. Denton Cooleyだったのです！！

いつまで経っても、外国人の名前を覚えるのが苦手なMasaでした、、、。

お家も素敵でしたが、Famillyもとっても素敵でした。

I was invited to Dr. Brady's Lab home party this weekend. Dr. Brady is a colleague of Dr. Hogue, who is my research mentor in Johns Hopkins, and he established a foundation for our research with Dr. Hogue. He moved from Johns Hopkins to Texas Children Hospital 3 years ago, and I've seen him several times before. 

Many research colleagues, including medical students and doctors, were invited and they took their family as well. I really enjoyed the party with them, and I met a medical student, who worked at Johns Hopkins for summer student program a couple years ago. 

"Hey, Masa! What are you doing??"
"I'm doing Cardiovascular Surgical Fellow at THI starting this month."
"My grandfather used to work at THI as a surgeon."
 ",,,. What is your last name??"

OMG! His grandfather is Dr. Denton Cooley, who is the founder of THI and considered to be the most famous cardiac surgeon in the whole world! 

I'm still not good at remembering foreigner's names,,,. 

THIの一週間

Texas Heart Institute:THIでORに出入りするようになって、2週間が経ちました。
来週あたりからOn-Callも始まりそうです、、、。

THIでは、前日に翌日の手術予定症例が各Resident/Fellowに割り当てられます。
10部屋のCardiac ORと、2部屋のHybdrid ORで、毎日10−20例の心臓血管手術が行われています。
今週から、幸運にも開心術の第1助手も当ててもらえるようになりました！
（患者の右側は、まだまだ"Far side"ですが、、、）

THIでの一週間は、

Mon：late start dayで8:00に患者が入室
（ということで、6:00〜Fredaの早朝英会話レッスン）
Tue：通常通り7:00入室
Wed：6:00〜Cardiac Surgery Educational Lecture
（text bookは"Cardiac Surgery" by Kirklin 4th edition）
Thu：7:00〜M&M (morbidity and mortality) conference 
Fri：通常通り7:00入室
Sat/Sun：On-call以外はお休み！

術後管理は麻酔科医と循環器内科医がメインで行います。
基本的には手術室でのHands-on trainingに集中できる、という外科医にとっては幸せな環境です。
On-callはResidentとFellowが組んで、4−5日に一回の割合で回ってきます。
（当然、当直手当は付きませんし、外勤もありません、、、）

とにかく、手術中のSurgeonとのCommunicationも含めて、まだまだ不安がいっぱい、、、。
が、泣き言言っていても誰も助けてくれないので、一生懸命やるしかない！！



I've been at THI for 2 weeks right now. Elective cases are distributed a night before surgery to residents and fellows. Fortunately, a few open heart surgical cases have been distributed to me this week. Unfortunately, I will be involved in an on-call schedule starting next week. On-call is likely to come every 4-5 days.

In ICU, Anesthesiologists and Cardiologists are taking care of postoperative patient mainly. So basically, residents and fellows can concentrate on surgical hand-on training in OR. 

Anyway, I still am anxious about my English communication, especially with surgeons during operations. Although nobody can help me in OR, I'll do my best, and try to have fun!!

J-1 visaについて

今回はUS visaについて。

自分は2010年に研究留学のために、2年間有効のJ-1 visaを取得しUSに入国しました。
その後、研究の1年間の延長を決め、2012年にCanadaでJ-1 visaを更新しました。
そして今回は、臨床留学のために、日本で再度、J-1 visaを取得しました。

ご存じの方もいらっしゃると思いますが、J-1 visaには2種類在ります。
正式名称はよく分かりませんが、J-1 researchとJ-1 clinical visaです。
USで臨床を行うためには、J-1 clinical visaが必要となります。
という事で、研究留学後に臨床留学をする場合はJ-1のcategory changeが必要になります。
調べるまでは、J-1のcategory changeは難しいと思っていましたが、実際には問題在りませんでした。

違ったのは、自分の場合、日本への帰国が確認されるまで、ECFMGからDS-2019が発行されませんでした。
なので、日本到着後にPassportのstump pageを送り、DS-2019を日本に送ってもらいました。

また、自分の理解している限り、J-1 visaはcategoryに関わらず、合計7年までです。
自分場合、既に3年間”J-1 research”だったので、”J-1 clinical”は最長で4年までですかね。
それと、J-1 clinicalはprogramの年数に関わらず、1年毎に更新が必要です。
さらに、いわゆる、”2-year rule”が適応となります。

”USで研究し、その後臨床したい！”と思っている、やる気のある心臓外科医の参考になれば幸いです。

ああ、めんどくせえぜ！

”Patient first” と口にする外科医は大勢いますが、
それがどういうことなのか、を彼を通して知ることができました。

Professor of Cardiac Anesthesiology
ResearchのBossで、ほとんど毎日顔を合わせていましたが、
3年間で怒られた事は一度もありませんでした、、、。
常に前向きに議論する”教育者”でした。
自分もいつかそう在りたい、、、。

Return from Japan to Houston!

VISAの申請も無事に済み、先週日本からHoustonに戻ってきました！
妻と子供達は、もう暫く日本を堪能してから再渡米の予定です。

自分は1年半ぶり、妻と子供達にとっては3年ぶりの日本、、、。
ということで、胃薬飲んで日本の食を堪能してきました！

大忙しでしたが、家族、親戚、そして友人や恩師の先生方と貴重な時間を共にする事が出来ました。
しかし、今回、残念ながら会う事が出来なかった友人もたくさんいました。
そんな皆さん、どうかHoustonに遊びに来て下さいな！！

ということで、先週からTexas Heart Institute:THIでの心臓外科研修も始まりました！
始まったといっても、まだ事務手続、健康診断や各種Orientationなどなどで、実際にはまだ一度しか手術室には入っていません。
が、どさくさに紛れて、第2助手として、Surgical Loupeを付けて、手術に入ってしまいました！
ああ、3年ぶりの心臓手術、今は、それだけで幸せ、、、。

さあ、家族が来るまでは寂しいですが、通勤用のBikeを買って、Runningして、またIKEA行って、、。
くそ暑いHoustonの夏を、そしてTHIでの手術修行をEnjoyするぞい！！

I've just returned from Japan to Houston last week! My wife and kids are still enjoying Japan a little longer. I could enjoy to see my family, friends, mentors in Japan, and to eat a lot of Japanese food!

The training at THI has just began from last week, but most of time was spent for orientations, paper works, health check, and electrical medical record lesson so far. But I had a chance to participate in cardiac surgical case with my surgical loupe. OMG! I felt so happy as it past 3 years since I did last time!

Anyway, I try to do my best to enjoy crazy hot Houston summer and cardiac surgical training at THI without my sweet family.
