しかし、こんなに慎重に行ったのは、医者1年目で、上級医に見守られながら、初めてICUで挿入した時以来でした、、、。
自分がAssistantとして開閉胸をした患者さんでした。
手術後のレントゲンで気胸が発見されました。
患者さんを見て、胸腔ドレーンを挿入した方がいいなあ、と思いました。
さあ、ここからが大変！
まずAttendingに報告。
”Masa、ドレーン挿入できる？”
”はい。”
”じゃあ、お願いね。”
そこに別の若手Attendingがたまたま通りかかる。
"Masa、一人で大丈夫？"
”じゃあ、一応、Chief Residentと一緒にします。”
”助けが必要だったら、いつでも連絡してね！”
ということで、Chief Residentに連絡。
"すぐ行くから始めてて！"
てなわけで、ICUの看護師さん達と胸腔ドレーン挿入の準備を開始。
”Masa、手伝うから大丈夫だよ！”
"Masa、今、人集めるからね！"
何だかんだで、胸腔ドレーン挿入ごとき？に5人以上の看護師が集合。
無事に胸腔ドレーンを挿入した時には、歓声と拍手が起こりそうな勢いでした。
で、挿入後のレントゲンを確認して、Attendingに報告。
たまたま通りかかった若手Attendingにも一応報告。
結局忙しくて来られなかった、Chief Residentにも一応報告。
皆一様に、"Masa、GOOD JOB！！"
その後、家族にお話をして同意書にSignをもらって一件落着。
言われた事を、確実に安全にこなして、周囲の信頼を得ていくのが大切なのは、日本でもアメリカでも同じ。
と、口で言うのは簡単だけど、ああ、うまくいって良かったよお！

I inserted the chest drain tube at ICU. I've inserted the chest tube many times in Japan. However, I've never been so nervous before.
After I discovered the pneumothorax, first of all, I had to inform it to the attending surgeon.
"Masa, Can you insert the chest tube confidently?"
"Yes, Sir."
"OK."
Another young attending surgeon was there by chance.
"Masa, just let me know if you need any help!"
I just let the chief resident know that I'm going to insert the chest tube.
"Masa, go ahead. I'm going to come soon!"
I started preparation with ICU nurses.
"Masa, you'll do just fine!"
More than five ICU nurses watched me attentively during procedure.
I could almost hear the cheer when I finally inserted the chest tube.
Anyway, everybody said "Masa, GOOD JOB!!"
It's really important for the physician to complete the task safely and surely, and it can lead to have other's trust either in Japan or US. I knew it, but it's tough. Hew,,,.
